1. Cololectomy (대장절제술; 결장절제술)

---

: Colectomy는 대장의 전체 또는 일부를 제거하는 수술

: 대장의 일부인 결장은 소화관 끝에 있는 긴 관 모양의 기관

: 대장절제술은 결장에 영향을 미치는 질병 및 상태를 치료하거나 예방하기 위해 필요할 수 있음
: 대장절제술에는 다양한 유형이 존재
    Total colectomy -  대장 전체를 제거
    Hemicolectomy - 대장의 오른쪽 또는 왼쪽 부분을 제거

 

 

 

2. Hemicolectomy (대장반절제술)

---

1) Right hemicolectomy

: 대장의 오른쪽을 제거하고 소장을 대장의 나머지 부분에 부착 (ileocolic anastomosis)

: ICA(ileocolic artery), RCA(Right colic artery) 및 MCA(Middle colic artery) 우측분지를 결찰

 

2) Extended right hemicolectomy

: Right hemicolectomy와의 차이점은 MCA의 origin에서 혈관을 결찰 
: 대장암이 Liver flexure(간만곡부)이나 횡행결장에 위치해서, 충분한 절제연(resection margin)이 필요한 경우

 

 

3) Transverse colectomy

: 대장의 중간 부분(횡행 결장)을 제거하는 것

: 양성 또는 악성소견 여부에 따라 절제 범위가 달라짐

 

4) Left hemicolectomy

: 대장의 왼쪽과 중간 일부를 제거하고 나머지 부분을 부착
: IMA(Inferior mesenteric artery)의 origin 동맥을 결찰 및 MCA의 좌측가지를 결찰

 

 

5) Anterior Resection (AR)

: 아래 표시된 D에서 F까지 절제하고 anastomosis

: 과거에는 Low Anterior Resection에 비교하여 'High Anterior Resection'이라 불리기도 함

 

 

6) Low Anterior Resection (LAR)

: 아래 표시된 D에서 G까지 절제하고 anastomosis

: AR과 LAR를 Peritoneal reflection를 기준으로 분류됨

    AR - Peritoneal reflection의 상방은 Intraperitoneal region

    LAR - Peritoneal reflection의 하방은 Extraperitoneal region

 

 

7) Abdominoperineal resection

: 아래 표시된 D에서 H까지 절제하고 anastomosis

: 괄약근을 침범하였을 경우 항문을 포함해 직장, 골반저근육을 모두 절제

: 회음부를 봉합하고 장루(Colostomy)를 형성

 

 

8) Hartmann's procedure (Hartmann's operation)

: 아래 표시된 A(절제), B(봉합), C(장루) 순서로 수행

: 대장의 절제가 필요한데, 응급으로 폐쇄를 해결해야할 때 사용
: 절제한 부분의 원위부는 복강 내에서 봉합한 상태로 두는 Hartmann's pouch(아래 B)가 특이적
