Child-Pugh Score

: 간경변의 만성 간질환 및 간이식 필요성의 예후를 평가하는 점수 체계

: 간 질환의 중증도와 그에 따른 예상 생존율을 제공
: Child-Pugh classification, Child-Turcotte-Pugh (CTP) calculator, Child Criteria 등으로도 불림

 

 

 

Child-Pugh Score 계산 과정

: Child-Pugh 점수는 간 질환의 5가지 임상 척도를 채점하여 결정

     hepatic encephalopathy(간성뇌증) - 간질환에 의한 뇌질환

     ascites(복수) - 복강 내 액체

     total bilirubin(총빌리루빈) - 헤모글로빈 분해로 생기는 담즙 화합물

     serum albumin(혈청알부민) - 간에서 생성되는 단백질
     prothrombin time or INR - 혈액 응고 시간

: 각 측정값에 1, 2, 3의 점수가 할당되며, 3이 가장 심각한 것
: 예를 들어, 복수 결과가 "중등도/이뇨제 불응성"인 경우, 해당 측정값은 3점
: 모든 점수가 총합하여 얻은 결과가 Child-Pugh score

Class A : 5~6점, 가장 덜 심각한 간질환, 1~5년 생존율 95%
Class B : 7~9점, 중증도의 간질환, 1~5년 생존율 75%

Class C : 10~15점, 가장 심각한 간질환, 1~5년 생존율 50%

 

 

 

Encephalopathy grade

: Child-Pugh score에서 Encephalopathy grade는 West Haven Criteria를 기준으로 함

 

 

 

MELD score

---

: MELD(Model for End-tage Liver Disease) score는 간 이식을 위한 성인 환자의 우선 순위 결정에 사용

: 사망 위험과 긴급성을 나타내는 심각도의 지표 (얼마나 긴급하게 간 이식이 필요한지)

 

 

 

PELD score

---

: PELD(Pediatric End-stage Liver Disease) score는 12세 미만 어린이의 MELD score

: MELD score와 마찬가지로 간 이식 환자의 우선 순위 결정에 사용
